The following groups of Chicago Police Department employees are required to file a Statement of Financial Interest with the City of Chicago Board of Ethics [BOE]. Members required to file are:

  • All Command Staff Members
  • Captains of Police
  • All Lieutenants of Police – Now including Lieutenants Promoted in 2017
  • Selected Non-Represented Civilian members.

Each member who is required to file should have received an email from the BOE at their department email address with instructions for filing. Filings are due by close of business 31 May 2017. Failure to file will result in a daily fine of $250.00. Affected members can access the BOE Electronic Financial Interests System at http://webapps.cityofchicago.org/efis/index.htm. For most filers, your login ID and password is the same one you use to log in to your computer. If you can’t log-in with those credentials, try using your City of Chicago Employee ID number as the username, and your last name (all CAPS) and the last 4 digits of your social security number (no spaces) as your password (e.g. SMITH1234). If you still have problems logging in you may contact the Board of Ethics at 744-9660. Members may also utilize the paper form for submitting their statement [attached PDF]. If utilizing the paper form please allow sufficient time for the submission to reach the Board of Ethics before filing deadline. Please be aware that filing the City of Chicago Statement of Financial Interest is in addition to the recently completed Cook County Clerk Statement of Economic Interests filing. Sworn members below the rank of Lieutenant and civilian members who were not notified via email by the Board of Ethics are not required to file. If you have any questions about logging in or how to answer specific questions, please contact the Board of Ethics at 312-744-9660.
